Why coverage varies by county
ACA Marketplace and Medicare Advantage plans are priced and sold by county-based rating areas, which means the same insurer can offer a completely different premium, network, or plan lineup just a few miles away across a county line. Carriers decide which counties to enter each year based on the local provider networks they can contract with, which is why a fast- growing suburban county might suddenly gain several new carriers in a single year while a neighboring rural county sees little change.
Metro vs. rural coverage differences
Texas's largest metro counties — Harris, Dallas, Tarrant, Bexar, and Travis among them — generally have the deepest carrier competition and the widest choice between HMO and PPO plan types. Smaller and more rural counties often have fewer carriers active at all, which can mean higher premiums and narrower networks, making it especially important to confirm your specific doctors are covered before enrolling. Planning a move within Texas?
